When we talk about skin ageing, most people think about collagen and elastin. While these proteins are essential, they are only part of a much larger and more sophisticated system known as the extracellular matrix (ECM) — the skin’s structural support network.

Understanding the extracellular matrix helps explain why skin changes with age, why certain treatments work, and what we can do to preserve healthy, resilient skin for longer.

What Is the Extracellular Matrix?

The extracellular matrix is a complex network of proteins, sugars, and signalling molecules that surround skin cells. You could think of the ECM as a building, with collagen acting as the steel framework that holds the skin together while also allowing cells to communicate, repair, and regenerate.

The ECM is found mainly within the dermis — the deeper layer of the skin — and is produced by specialised cells called fibroblasts.

What Happens to the ECM as We Age?

From our mid-20s onwards, the production of collagen, elastin, and hyaluronic acid gradually declines. At the same time, environmental damage accelerates ECM breakdown.

This leads to:

  • Fine lines and wrinkles

  • Loss of skin firmness

  • Reduced elasticity

  • Thinning skin

  • Dehydration

  • Slower healing

  • Dull or uneven texture

One of the major drivers of ECM ageing is chronic inflammation and oxidative stress caused by external factors such as UV exposure and pollution.


The Biggest Threats to the Extracellular Matrix


Sun Exposure

Ultraviolet (UV) radiation is one of the most damaging factors for the ECM. UV light activates enzymes which break down collagen and elastin fibres.

This process, known as photoageing, is responsible for much of the visible ageing we associate with sun damage. If you're a patient of mine you'll have heard me promote hat wearing. It's one of the most simple and effective ways of protecting your skin! And costs nothing.

Smoking

Smoking reduces oxygen delivery to the skin and increases free radical damage, accelerating collagen degradation.

Poor Nutrition

Diets high in sugar and processed foods can contribute to glycation — a process where sugar molecules damage collagen fibres, making them stiff and fragile.

Chronic Stress

Long-term elevated cortisol levels can impair collagen synthesis and increase inflammation.

Environmental Pollution

Pollutants generate oxidative stress within the skin, contributing to premature ECM breakdown.


How Can We Protect Our Skin and the Extracellular Matrix?


Protecting the ECM requires a combination of daily skincare, healthy lifestyle habits, and evidence-based aesthetic treatments.

Daily SPF Use

Broad-spectrum sunscreen is the single most effective way to preserve collagen and prevent ECM degradation.

Consistent SPF use helps reduce UV-induced collagen breakdown and protects against premature ageing. Team that up with regular hat wearing in bright sunshine and your future self with thank you.

Antioxidants

Topical antioxidants such as vitamin C help neutralise free radicals before they damage collagen and elastin. Choosing an effective Vitamin C preparation can be challenging. I would say a premium product would work best to ensure the active ingredient is potent enough.

Vitamin C also supports collagen synthesis, making it an important ingredient in preventative skincare.

Retinoids

Retinoids are among the most scientifically studied ingredients in skincare. They stimulate fibroblasts, encourage collagen production, and improve overall ECM quality over time.

Hydration and Barrier Support

Maintaining a healthy skin barrier helps reduce inflammation and support optimal ECM function.

Ingredients such as ceramides, hyaluronic acid, and peptides can help maintain hydration and skin resilience.


Aesthetic Treatments That Support the ECM

Modern aesthetic medicine increasingly focuses on improving skin quality by stimulating the extracellular matrix rather than simply masking signs of ageing.


Treatments that may support ECM regeneration include:

  • Microneedling

  • Collagen-stimulating injectables such as Sculptra

  • Polynucleotide treatments

  • Injectable Skin boosters


These treatments work by triggering controlled repair processes that encourage fibroblasts to produce new collagen and other ECM components.
